Ethics Forms

All projects MUST have ethics approval documented before scanning hours will be released on the web scheduler. For healthy adult subjects, investigators can submit applications to the intradepartmental Ethics coordinator, Essi Viding. An ethics packet (zipped archive) is available here. For paediatric or clinical participants, separate Ethics cover must be obtained from the relevant board.

Applications with an electronic signature of the PI (all in doc format please) should be e-mailed to Essi Viding (e.viding@ucl.ac.uk). All applications should include the data protection number, and a pdf copy of the risk assessment form signed by all researchers, as well as all standard scanning documents. These forms can be submitted at any time. It may take up to four weeks to have your application reviewed, please make sure that you submit your application in good time and do not chase it up unnecessarily.
